Community Nursing Team is the States of Guernsey's community nursing service, delivering individualised home nursing care around the clock. Based at Castel Hospital, the team provides home nursing, palliative care, tissue viability and lymphoedema nursing, Admiral Nurse dementia support, rapid response and reablement, community disability support, and urology/continence care. The service operates an open referral system accepting contacts from health professionals, patients, relatives, carers, and other agencies, with the exception of the Rapid Response Team which operates separate access protocols. —
