About

Children's Community Nursing Service is a States of Guernsey Health, Social Care and Wellbeing (HSC) service delivering home-based nursing care for children and young people from birth to 18 and their families. The team of four nurses and one nursing associate, based at Lukis House, St Peter Port, provides post-discharge follow-up, Type 1 Diabetes management, neonatal transition support, continence care, and multi-disciplinary care for children with complex needs. Referrals are accepted via Health Visitors (under 5s) or School Nurses (over 5s).
